How Do I Choose a Provider ?

There are a couple of ways to choose your provider. Here’s how:
   
ONE:




If you already know who or what agency you would like to provide your service (they must be a certified agency or individual provider), tell your SSA (Service & Support Administrator) and he or she will help you contact the provider to see if they are able and willing to provide your service.

   
TWO:
If you are not sure who you would like as your service provider, do the following:
   
  A. Review the Provider Handbook to see the providers that are currently providing the supports you are looking for and ask your SSA to help you contact some of the providers to see if they are interested in meeting you to discuss your service request.

B. You can work with your SSA to develop aprofile/description of the service(s) you are looking for, which may include:                             
Type of services requested                             
Number of support hours                             
Location of service(s)

The County Board can then place your profile on the County Board website for all providers to see. Interested providers can then contact your SSA if they are interested in providing the services you need or want.

C. If you know someone who would like to provide your service(s) but he or she is not a certified provider, contact Lavonne Meyer at 740-695-0460, ext. 301. You may also call the Ohio Department of DD Support Center at 1-800-617-6733 and choose Option 3.
